Job summary

Mass General Brigham Community Physicians, Inc. seeks a Patient Navigator to work on-site approximately four times per week across East Region practices in Boston, MA.

The navigator will assist patients and families, coordinate resources and help streamline access to care to improve outcomes. Key duties include advocating for patients, serving as a liaison among providers, coordinating appointments, and educating patients about treatment options and self-care.

Qualifications

• Bachelor's Degree required; relevant experience may be accepted in lieu of degree.
• 1+ years of experience in social work, community health, case management or related field required, preferably in a clinical setting.

Responsibilities

• Advocate for patients and their families, ensuring their needs are considered in care planning.
• Serve as liaison between patients, healthcare providers and other stakeholders.
• Coordinate patient care services, appointments and follow-up plans with the interdisciplinary team.
• Assist patients in understanding and adhering to their care plans.
• Identify and connect patients with healthcare and community resources.
