Qualifications

  • High School diploma or equivalent.
  • Minimum 4 years of relevant work experience.
  • Ability to communicate effectively in both oral and written form.
  • Ability to handle difficult and stressful situations with professional composure.
  • Ability to maintain effective interpersonal relationships.
  • Ability to understand and follow instructions.
  • Ability to process and handle confidential information with discretion.
  • Must be able to route communications to the appropriate persons.
  • Must have strong computer skills.
  • Must be able to be redirected as needed for cross coverage and emergent tasks.

Responsibilities

  • Identify type and urgency of medical need.
  • Conduct patient evaluations to develop an effective treatment plan.
  • Ensure access to insurance and social services for patients.
  • Maintain knowledge of insurance referral requirements.
  • Provide reminders and act as liaison between patients and providers.
  • Communicate appointment and payment policy details to patients.
  • Identify and resolve barriers to care.
  • Intervene as liaison/advocate for patients, physicians, and staff.
  • Educate and counsel patients on health and life issues.
  • Maintain knowledge of community resources and programs.
  • Assist in building partnerships with local physicians and referral sources.
  • Assist lower-level Patient Navigators as needed.
  • Adhere to University and unit-level policies and procedures.
