Job summary

HopeHealth is seeking an experienced Palliative Nurse Practitioner to treat patients in inpatient and community settings. This per-diem role requires weekend shifts at major hospitals in Providence, RI, with flexibility to cover other facilities as needed.

The NP will collaborate with treating physicians to relieve pain and other symptoms, communicate serious news, and help establish goals of care. The position emphasizes teamwork and expert hospice/palliative care leadership.

Qualifications

  • Master’s degree in Nursing or Doctorate in Nursing is required.
  • Nurse Practitioner certification (Family/Geriatric/Adult) required in RI.
  • RI/MA RN and NP/PA licenses required.
  • Prescriptive authority licenses (RI/MA) and DEA license required.
  • Driver’s license with insured vehicle.

Responsibilities

  • New consults and follow-up consults for seriously ill adults in inpatient/outpatient settings.
  • Complete pain assessments and pain management recommendations for seriously ill adults.
  • Assess and manage non-pain symptoms like nausea, fatigue, dyspnea, anxiety.
  • Assess disease trajectory and formulate prognosis with treating physicians.
  • Serve as a seasoned consultant, fostering relationships with providers.
  • Triage weekend consults and arrange follow-up for less urgent issues.
  • Make recommendations on further treatment and end-of-life options.
  • Coordinate care with patients, families, providers, and facilities.

About This Palliative Nurse Practitioner (NP) Position

HopeHealth is seeking an experienced Palliative Nurse Practitioner for its affiliate HopeHealth Hospice & Palliative Care to treat patients inpatient and in the community.

This is a per-diem position that is not eligible for benefits. To stay active with the role, we require a minimum of two weekend shifts per month (Saturday or Sunday) working at one of two large academic hospitals in Providence, RI. The ideal candidate will have added flexibility to pick up shifts at community hospitals during the week, filling in planned and unplanned absences on HopeHealth’s inpatient palliative care teams that are embedded in these hospitals.

The Palliative Care Nurse Practitioner works in collaboration with the referring provider and appropriate consultants to provide relief of suffering for patients and families facing serious illness. They will provide assessment and recommendations to relieve burdensome symptoms like pain, dyspnea, anxiety, fatigue. They will be able to communicate serious news and help patients and families establish goals for care compassionately and clearly.

The palliative Nurse Practitioner needs to be highly collaborative, working cooperatively with patients, families, referring providers, and hospital staff, serving as an expert in hospice and palliative care.

Salary Range:$120,000 – $145,000 [per year / DOE (Depending on Experience)]

Duties / Responsibilities
  • New consults and follow up consults for seriously ill adults in a variety of inpatient and outpatient settings including but not limited to homes, assisted livings, nursing facility, hospitals, clinics.
  • Complete pain assessments and pain management recommendations for seriously ill adults.
  • Assesses and makes recommendations to manage other non-pain symptoms including nausea, fatigue, dyspnea, anxiety.
  • Assesses patient’s disease trajectory and co-morbidities and, in conjunction with treating physicians, formulates prognosis
  • Works as a seasoned consultant, fostering positive and collaborative relationships with referring medical providers or entities
  • Especially on weekend shifts, triaging consults, prioritizing urgent issues and arranging for appropriate follow up of less urgent issues.
  • Based on prognosis and formally elicited patient and family goals, makes recommendations regarding both further treatment and end of life options
  • Counseling and Coordination of care with patients, families, providers, facilities asImproves clinical outcomes by conducting research or applying the research findings of others.
Education / Experience
  • Graduated from an accredited master’s degree program in Nursing and/or Clinical Doctorate program in nursing.
  • Certified as a Family/Geriatric or Adult Nurse Practitioner and certified as a Registered Nurse Practitioner in the state of RI required.
  • Minimum of one to three years' experience as nurse practitioner or physician's assistant, preferably in the field of Palliative Care. Minimum of 2 years' experience in geriatrics, acute care, hospice, or oncology.
  • Experience with pain and symptom management is highly preferred.
  • Knowledge of third-party reimbursement procedures and requirements is helpful.
  • Experience in hospice and palliative care is highly preferred and with certification as advance practice in hospice and palliative care. Experience in primary care within a hospital or nursing home preferred.
  • Current RI and MA licenses as RN and Registered Nurse Practitioner or Physician's Assistant.
  • Current RI and MA licenses for prescriptive authority, RI and MA controlled substance license, DEA license.
  • Valid driver's license with and automobile that is insured in accordance with state and/or agency requirements and is in good working order.
