Qualifications

• Experience with interdisciplinary care teams and caregiver education.
• Proficiency with electronic medical records and Microsoft Office applications.

Responsibilities

• Conduct comprehensive occupational therapy evaluations, treatments, and interventions for PACE participants.
• Develop individualized care plans and implement therapeutic interventions.
• Collaborate with the Interdisciplinary Team to achieve participant-centered goals.
• Supervise and evaluate COTAs, support competency assessments.
• Coordinate home modifications and ordering of durable medical equipment.
• Document participant progress in the medical record and monitor treatment effectiveness.
