Job summary

Socket.dev is seeking a Senior Service Coordinator in Minnesota to respond to seniors’ needs, assess eligibility for services, and coordinate food service for congregate and home-delivered meals. Qualifications

• Two-year degree in social work, nutrition, or related field plus one year of related experience or equivalent combination of education and experience.
• Previous experience working with social service programs related to the elderly and coordinating food service preferred.
• Serv Safe certification preferred.

Responsibilities

• Manage caseload of clients and conduct 6-month check-ins via home visits or phone to discuss needs and connect with resources.
• Coordinate total food service for congregate dining and home-delivered meals, including daily meal orders and changes.
• Recruit, supervise and train volunteers; engage local community and faith-based orgs to recruit volunteers.
• Ensure coverage of volunteers daily for sites and routes, track hours and mileage.
• Assist clients with SNAP, EAP, Food Shelf and other applications; conduct outreach to increase caseload.
