Job summary

Socket.dev seeks a Senior Service Coordinator to support Senior Nutrition services at Shakopee/Chaska sites. The role coordinates congregate and home-delivered meals, manages a volunteer workforce, and conducts community outreach to grow caseload and partnerships.

Responsibilities include client assessments, intake data entry, and helping clients access SNAP and related programs, with coordination to ensure daily operations run smoothly.

Qualifications

• Two-year degree in social work, nutrition, or related field plus one year of related experience or equivalent combination of education and experience.
• Experience with social service programs for the elderly and coordinating food service preferred.
• Serv Safe certification preferred.

Responsibilities

• Manage caseload and complete a 6-month check-in with clients to discuss needs and resources.
• Coordinate total food service for congregate dining and home delivered meals.
• Recruit, train, and supervise volunteers for meals and routes.
• Record daily meals served and enter counts in tracking system.
• Assist clients with SNAP, EAP, Food Shelf and related applications.
• Conduct outreach and build partnerships with community organizations.
