Job summary

Mid Florida Community Services, Inc. in Brooksville, FL is seeking a Health/Nutrition Program Assistant to support the Program Manager in daily oversight of nutritionally balanced meals, nutrition education, and screenings for seniors aged 60+.

Services are provided at dining facilities including senior centers and community sites. The role includes maintaining client records, completing intake, training meal-site staff, creating monthly calendars and education materials, monitoring meal counts

Qualifications

Associate’s Degree in Human Services related field preferred.
One year experience working with elder population. An equivalent combination of education/experience may be considered.
Valid Florida driver’s license and valid insurance. Subject to background checks for working with a vulnerable population.

Responsibilities

Oversees client record keeping including assessments, reassessments and eligibility determination.
Completes intake forms and refers clients to appropriate resources.
Coordinates and trains meal site staff/volunteers in health/safety standards and state/federal guidelines.
Provides monthly activity calendars and nutrition education materials to active clients.
Monitors and projects weekly/monthly meal counts and ensures proper paperwork for each meal site.
Assists with donations and meal delivery as needed.
