Oak Street Health in Jennings, MO is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare. The role collaborates with a care team to deliver longitudinal care and coordinate with physicians, specialists, and care managers.

Full-time, Monday–Friday, 8am–5pm, with a salary range of $129,000–$148,000 and comprehensive benefits. US work authorization is required and a valid NP license to practice in Missouri.

Qualifications

• MSN/NP credential and national certification in one of listed specialties.
• Active, non-probationary state NP license
• Active DEA license
• US work authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team to ensure appropriate screenings.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
