The role is full-time and requires NY NP license, AANP/ANCC certification, DEA license, US work authorization, and a patient-centered approach. Strong communication, ethics, and commitment to compassionate care are essential.

Qualifications

• Bachelor's Degree in Nursing required.
• AANP or ANCC national NP certification.
• Active NY NP license.
• DEA license for controlled substances.
• US work authorization.
• Strong interpersonal and adaptable skills.
• Commitment to serving marginalized populations.

Responsabilités

• Provide tailored health education to seniors and underserved patients.
• Develop and communicate comprehensive treatment plans.
• Order and interpret labs and imaging; adjust plans accordingly.
• Maintain compliant EMR and regulatory documentation.
• Collaborate with clinical leadership to meet healthcare goals.
• Support value-based care and quality improvement initiatives.
• Ensure high patient satisfaction and patient-centered care.
• Participate in interdisciplinary team meetings and care conferences.
• Complete regulatory visits (monthly, quarterly, etc.).
• Coordinate medication management with pharmacists.
• Diagnose and manage chronic and acute conditions with patient safety in mind.
