Key Responsibilities

• Providing tailored health education to patients and families to promote understanding of chronic conditions and preventive health measures.
• Developing and articulating comprehensive treatment plans to patients, families, and other healthcare providers, ensuring clarity and understanding.
• Ordering and interpreting laboratory tests and diagnostic studies, making informed adjustments to treatment plans as necessary based on results.
• Maintaining accurate and compliant electronic medical records (EMR) to ensure continuity of care and adherence to regulatory requirements.
• Collaborating with local and regional clinical leadership to align strategies and achieve established healthcare goals.
• Actively supporting initiatives focused on value-based care and continuous quality improvement.
• Fostering high levels of patient satisfaction through exceptional customer service and patient-centered care.
• Practicing ethically and in strict accordance with the professional standards set forth by relevant licensing boards and regulatory bodies.
• Participating in interdisciplinary team meetings and care conferences, contributing expert insights for comprehensive patient care coordination.
• Completing all regulatory required visits in a timely manner, including monthly, quarterly, and acute care assessments.
• Addressing pharmacy-related requests and collaborating with pharmacists on medication management to ensure optimal prescribing practices.
• Evaluating, diagnosing, and managing both chronic and acute health conditions while ensuring patient safety and compliance with care standards.

Patient Population / Client Focus

Your primary focus will be on senior patients and low-income populations within assisted living and skilled nursing facilities. Qualifications

• Possession of a Bachelor’s Degree in Nursing.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, unrestricted Nurse Practitioner license in the state of New York.
• A current DEA license for controlled substance prescribing.
• Authorization to work in the United States.
• Strong interpersonal skills and the ability to adapt to a changing work environment are essential.
• A commitment to serving marginalized populations with compassion and respect.

Preferred Experience

• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a nurse practitioner certificate program.

Compensation and Benefits

The compensation for this position ranges from USD 150,000 to 160,000 annually, commensurate with experience.
