Job summary

Community Physicians is seeking a Board-certified NP or PA to provide outpatient geriatric care in a collaborative, supportive clinic. You will co-manage a defined panel of older adults with a lead physician and work with a multidisciplinary team to deliver high-quality, patient-centered care.

You’ll perform preventive screenings, review medications, and contribute to care planning across the continuum, including post-acute settings. Strong communication and mentorship ethos are valued.

Qualifications

• Board-certified NP or PA with licensure in Illinois.
• Interest in geriatrics is preferred.
• Ability to work across multiple care settings.
• Excellent communication with patients and families.
• Minimum 3 years as a Nurse Practitioner.
• Independent clinical decision-making in a fast-paced setting.

Responsibilities

• Co-manage a defined panel of geriatric patients with lead physician.
• Collaborate with a multidisciplinary team in outpatient clinic.
• Perform preventive screenings and discuss long-term goals of care.
• Review medication lists and order labs/diagnostics.
• Maintain accurate patient records and charts.
• Provide care across continuum including post-acute settings.
• Engage in leadership and clinical development opportunities.
