Job summary

Vis-À-Vis Health seeks a full-time on-site Nurse Practitioner to provide comprehensive primary care in a Harlem nursing facility. The NP will assess, diagnose, order tests, and develop evidence-based care plans for residents.

The role requires an active NY NP license with board certification and experience in geriatrics or long-term care. You will collaborate with facility staff, physicians, and families, while documenting care and participating in quality improvement.

Qualifications

• Master’s degree in Nursing (MSN) or higher.
• Active Nurse Practitioner license in New York State.
• Board certification as NP (Adult, Family, or Gerontology).
• Experience in geriatrics, long-term care, or community health.

Responsibilities

• Provide comprehensive primary care including routine and acute assessments.
• Diagnose conditions, order and interpret diagnostic tests, and develop care plans.
• Prescribe and monitor medications; manage chronic diseases.
• Conduct follow-up visits and coordinate care with physicians and facility staff.
• Document encounters per regulatory and facility standards; educate patients and caregivers.
• Participate in quality improvement initiatives to improve patient outcomes.

Education
