Optum Home & Community Care is expanding its Senior Community Care (SCC) team to provide high-quality clinical care to patients in nursing homes, senior housing, and assisted living settings.

The Advanced Practice Clinician will conduct assessments, manage short-term rehab, and lead advanced care planning conversations while collaborating with an interdisciplinary team. A valid state license, DEA, and certification are required to practice.

Qualificações

• Active ANCC or AANP certification for NPs or NCCPA certification for PAs
• Active, unrestricted state licensure in the practicing state
• Active and unrestricted driver's license with access to reliable transportation
• Current/ability to obtain DEA licensure/prescriptive authority
• Ability to gain a collaborative practice agreement where applicable
• Ability to navigate varied care environments and perform job duties

Responsabilidades

• Conduct comprehensive assessments including medical history, meds reconciliation, vitals, and exams
• Support members during short-term sub-acute rehab episodes
• Ensure accurate documentation including ICD-10
• Collaborate with interdisciplinary care team
• Conduct advanced illness and care planning conversations
• Provide counseling and education to patients and caregivers
