MGA Homecare is seeking a Registered Nurse Clinical Case Manager to supervise home health personnel, ensure regulatory compliance, and coordinate patient care plans. The role involves guiding field staff, educating patients and families, and collaborating with the Clinical Supervisor and Director of Clinical Services.

Ideal candidates hold an active RN license, with BSN preferred, and have at least two years in home health or acute care.

Formación

• Current state license as a Registered Nurse (BSN preferred).
• Minimum of two years nursing experience in a Home Health or Acute setting preferred.
• Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
• Detail oriented, flexible, good organizational and time management skills.
• Knowledge of cost management for clinical services.
• Current CPR card and Current health certificate as applicable.
• Current state driver’s license and automobile insurance.

Responsabilidades

• Supervise home care personnel as assigned and ensure regulatory compliance.
• Coordinate implementation of each patient’s Plan of Care and communicate effectively.
• Instruct and re-educate on clinical skills/procedures; train field staff.
• Provide support and education to patients, families, or caregivers.
• Report occurrences to Clinical Supervisor/Director of Clinical Services.
• Develop and implement patient care plans under physician direction.
