Professional Case Management is seeking a Nurse Administrator to direct nursing operations and clinical staff in the home health setting. You will collaborate with the Regional Director, ensure regulatory compliance, and oversee staffing, documentation, and performance improvement to meet patient needs.

The role requires an active RN license, CPR, and 2+ years of nursing with at least 1 year in home care, including supervisory/case management experience.

Qualifikationen

• Must be a graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state of practice.
• Minimum 2 years nursing experience including 1 year in home care.
• 1 year supervisory/case management experience preferred.
• Current CPR certification.
• Licensed driver with clean driving record and insured.

Aufgaben

• Assist Regional Director and staff to manage services and improve quality.
• Ensure compliance with state regulations and credentialing.
• Plan, implement and evaluate client care.
• Coordinate and lead client care conferences as needed.
• Serve as on-site clinical resource to meet home care needs.
• Assist Staffing Coordinators with staffing and scheduling within scope of practice.
• Ensure adherence to policies and delegation guidelines.
• Participate in interview, selection and evaluation of clinical staff.
• Support performance-improvement initiatives and data collection.
