Medical University of South Carolina is seeking a Nurse Case Manager II who will coordinate a comprehensive plan of care to achieve clinical outcomes and promote a safe, timely discharge. This role oversees the plan of care across the care continuum to ensure efficient patient movement through the hospital system.

Requires a Bachelor's degree in Nursing and three years of RN experience, SC licensure or compact state, and BLS. Leadership and Case Management certification are preferred.
