MUSC Health in Charleston, SC seeks a Nurse Case Manager II to coordinate a comprehensive plan of care and ensure safe, timely discharge. This role integrates with the Case Management & Care Transitions team to move patients smoothly through the hospital system.

The position requires a BSN, RN licensure in SC or a compact state, and 3+ years of RN experience with preferred Case Management exposure. This salaried role follows a Monday–Friday schedule with occasional weekends and holidays.

Formación

• Bachelor of Science in Nursing (BSN) from an accredited program.
• Current RN license in South Carolina or compact state.
• Case Management experience preferred; leadership experience encouraged.

Responsabilidades

• Coordinate a plan of care to achieve clinical outcomes.
• Promote a safe, timely discharge and efficient patient movement through the hospital.
• Serve as clinical expert in discharge planning, resource utilization, and case management.
