Job summary

Kaiser Permanente is seeking an experienced registered nurse case manager in California to coordinate care for a caseload of low- and medium-risk patients. You will interview patients and caregivers, assess needs and goals, and develop comprehensive discharge and care plans in collaboration with the care team.

You will obtain authorizations as needed, supervise care coordination, and ensure alignment with regulatory guidelines while supporting quality outcomes and patient access to services.

Qualifications

• Bachelor's degree in Nursing or related field.
• California RN license required at hire.
• Minimum five years of nursing, case management or related experience.

Responsibilities

• Provide case management services to a caseload of low- and medium-risk patients.
• Interview patients and caregivers to assess needs and goals.
• Develop and implement discharge and care coordination plans.
• Attend case management rounds and update authorizations and diagnoses.
