Collaborate with clinicians to authorize services and coordinate resources for continuous, high-quality care. Required: Bachelor’s degree in Nursing or related field and CA RN license; minimum five years in nursing/case management.

Qualificações

• Bachelor's degree in Nursing or related field and five years of nursing/case management experience.
• California RN license required at hire; CCM preferred in the state of care.
• Experience coordinating care, authorizations, and dispositions with care teams.

Responsabilidades

• Interview patients and caregivers to assess needs, goals, and services.
• Develop and implement client-focused case management plans with regulatory alignment.
• Coordinate resources to ensure continuity and quality of care.
• Attend case management rounds and update authorizations and diagnoses as needed.
• Identify barriers to goals and discuss with patient and care team.
• Document cases in medical files and review benefits/services for patients and families.
• Connect patients with services and assist with access to care and benefits coverage.
