Qualifications

• Minimum two years as a Case Manager/Care Coordinator and two years of geriatric clinical practice in home health, hospice, hospital or long-term care setting.
• BSN or health-care related degree, or four years of related experience in case management.

Responsibilities

• Enhances day-to-day case/care management delivery and supports quality improvement in SNF facilities.
• Interprets utilization criteria, educates patients and families on Medicare and benefit rights, and ensures compliance.
• Facilitates efficient care transitions, coordinates resources, and maintains timely and high-quality care.
• Executes ongoing communication with patients, families, and care teams to address concerns and improve outcomes.
