Job summary

Responsibilities

• Plan, develop, assess and evaluate care for members.
• Collaborate with physicians and care team to develop plans of care.
• Recommend alternate levels of care per guidelines and ensure compliance.
• Assess high-risk patients for post-hospital planning.
• Develop and coordinate discharge plans to meet patient needs.
• Coordinate transfers and obtain necessary authorizations/approvals for outside services.
