Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver high-quality, value-based primary care to Medicare patients in a collaborative outpatient setting. You will work with physicians, nurses, and care managers to provide comprehensive preventive and chronic disease management within a patient-centered care plan.

The role emphasizes independent patient visits, coordinated care across the care team, and active participation in quality initiatives and promotions.

Qualifications

• Must hold MS in Nursing, DNP, or NP certificate.
• Active NP license (state) with no probation.
• Active DEA license.
• US work authorization.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Perform annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with specialists, facilities, and care managers.
• Lead population health initiatives with the care team.
• Assist with triage and outreach by phone.
• Educate patients on conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
