Job summary

Endeavor Health is seeking a Mental Health Clinical Therapist II for Deerfield, IL. This full-time position provides quality clinical services within a physician office or similar setting, as part of an interdisciplinary team.

The role includes developing treatment plans, conducting psychosocial assessments, and mentoring new staff while upholding compliance and ethical standards of care. Licensed clinicians with relevant credentials are preferred.

Qualifications

• Master’s degree in a recognized mental health field.
• Current Illinois licensure with a minimum of four letters or a Psych D which may include LCPC, LCSW or LMFT.
• CPR Certification issued by American Heart Association.

Responsibilities

• Provide individual, couples and family process-oriented counseling sessions as appropriate.
• Conduct psychosocial assessments by interviewing patients and significant others to identify treatment focus.
• Develop, implement and monitor the patient’s individualized treatment plan.
• Participate in regular clinical team meetings and contribute to case discussions on psychosocial needs.
• Coordinate care with referral sources and other providers as needed to ensure continuity of care.
