Job summary

Endeavor Health in Deerfield is seeking a Mental Health Clinical Therapist II FPA to join the interdisciplinary team in a physician office setting. The role provides individual, couples and family counseling, develops treatment plans and coordinates care with physicians and staff.

Qualified candidates hold a Master’s in a recognized mental health field and Illinois licensure with four letters (LCPC/LCSW/LMFT) or Psych D, plus CPR and CPI certifications.

Qualifications

• Master’s Degree in a recognized mental health field.
• Knowledge of developmental psychology, disease pathology and clinical needs of assigned population.
• Ability to maintain information which is highly confidential.
• Ability to summarize clinical information in writing and medical records.
• Ability to stay self‑controlled in volatile situations.
• Ability to set therapeutic limits and maintain boundaries with clients.

Responsibilities

• Provide individual, couples and family process-oriented counseling sessions.
• Develop, implement and monitor the patient's individualized treatment plan.
• Participate in regular clinical team meetings and case discussions.
• Inform attending physicians of progress and changes in emotional/mental status.
• Maintain accurate medical records including assessments and progress notes.
