Qualifications

• Master’s degree in a recognized mental health field.
• Current State of Illinois licensure with a minimum of four letters or a Psych D (LCPC/LCSW/LMFT).
• Ability to maintain information that is highly confidential.
• Ability to summarize clinical information via written records.

Responsibilities

• Conduct psychosocial assessments with patients and families.
• Coordinate care with referral sources and providers.
• Develop and monitor individualized treatment plans.
• Provide individual, couples, and family counseling.
• Participate in clinical team meetings and case discussions.
• Maintain accurate medical records and progress notes.
• Inform physicians about treatment progress and changes in status.
• Assist in transitions to inpatient services when needed.
• Utilize community resources for post-discharge care.
