Job summary

Endeavor Health is seeking a MH Clinical Therapist II in Addison, IL for full-time daytime hours. The role involves conducting psychosocial assessments, coordinating care with providers, and developing patient-centered treatment plans.

You will provide individual, couples and family counseling and participate in team meetings to address psychosocial, disease management and case management needs. Illinois licensure is required, with CPR certification.

Qualifications

• Master’s Degree in a recognized mental health field is required.
• Minimum of two years of experience in a behavioral health setting is preferred.
• Current State of Illinois licensure with LCPC/LCSW/LMFT or Psych D is required.

Responsibilities

• Conducts psychosocial assessments by interviewing the patient and family to identify treatment focus.
• Coordinates with referral sources and providers to inform enrollment and obtain collateral data.
• Develops, implements and monitors the patient’s individualized treatment plan.
• Provides individual, couples and family counseling sessions.
• Participates in clinical team meetings and contributes to case discussions on psychosocial needs.
• Informs physicians about psychotropic medication management and treatment progress.
• Transitions clients to inpatient care when needed for psychiatric hospitalization.
• Maintains medical records including assessments and progress notes.
• Describes therapeutic interventions for discharge planning and follow-up.
