The position requires a Bachelor’s in social work, Michigan licensure, and a Master’s degree with licensure for higher pay grades.

Qualifications

• Bachelor’s degree in social work required and Michigan licensure/certification required.
• Master’s degree in Social Work, Psychology, or Counseling with Michigan licensure.
• Experience with QMHP/CMHP and related CAFAS/PECFAS training preferred.

Responsibilities

• Assess consumer and family needs and arrange, coordinate, monitor and advocate for services through PCP and IPOS/FPOS.
• Address biopsychosocial status and system context to ensure delivery of recovery‑oriented services.
• Link consumers with external systems to provide additional resources aligned with performance standards.
• Provide intensive case management or therapeutic interventions as assigned; crisis support may be required.
