OU Health in Oklahoma City seeks a Medication Reconciliation Pharmacy Technician to obtain and verify patient medication histories across inpatient care settings under pharmacist supervision. Qualificações

• Associate's degree required.
• 5+ years of experience as a pharmacy technician.
• Active permit as a Pharmacy Technician by the Oklahoma State Board of Pharmacy within 6 weeks of hire; National Pharmacy Technician Certification (CPhT) required.

Responsabilidades

• Obtain complete home medication histories through patient and caregiver interviews.
• Verify histories with community pharmacies, prescribers, LTC facilities, and other entities.
• Document verified histories and allergies in the electronic health record accurately and promptly.
• Identify discrepancies between home meds and orders and communicate for resolution with pharmacists and providers.
• Collaborate with pharmacists, nurses, and clinicians to support reconciliation across transitions of care.
