Job summary

OU Medical Center seeks a Medication Reconciliation Pharmacy Technician in the Pharmacy Department. This advanced role obtains, verifies, and documents inpatient medication histories under the supervision of a licensed pharmacist to support safe transitions of care.

Requires Associate’s degree, at least five years of experience, Oklahoma Pharmacy Technician license within 6 weeks of hire, and CPhT certification.

Qualifications

Associate’s degree required.
At least five (5) years of experience as a pharmacy technician required.
Active permit as a Pharmacy Technician by the Oklahoma State Board of Pharmacy within 6 weeks of hire.
National Pharmacy Technician Certification (CPhT) required.
Proficiency with electronic health records and pharmacy documentation systems.

Responsibilities

Obtains complete home medication histories via patient interviews.
Verifies histories with community pharmacies and prescribers as needed.
Documents histories and allergy information in the EHR.
Identifies discrepancies between home meds and orders and notifies pharmacists.
Collaborates with pharmacists, nurses, and physicians to support reconciliation across transitions of care.
Maintains patient confidentiality and HIPAA compliance.
Supports QA initiatives to improve accuracy and safety.
