Qualifications

• Four years of post-high school science experience or equivalent pharmacy technician experience.

Responsibilities

• Triage patients for medication history collection based on complexity.
• Interview patients/caregivers and collect data for admission histories.
• Document allergy and medication lists in the EMR accurately.
• Coordinate with pharmacist staff and other healthcare professionals as needed.
• Independently author medication history notes within the EMR.
• Supervise pharmacy student interns and provide feedback on performance.
