Job summary

Trinity Health seeks a Medication Historian Technician to conduct patient interviews for comprehensive medication histories in pre-admission testing, emergency department or procedural areas. You will use standardized forms and access diverse sources to verify outpatient regimens and document them in the electronic medical record.

Qualifications

• Minimum education: high school diploma or equivalent.
• Minimum of 1 year pharmacy experience.
• Must become nationally certified within 1 year of employment.
• Strong communication and leadership skills.
• Accurate and detail oriented with exemplary time management.
• Knowledge of brand and generic medication names and dosing for top medications within 90 days of hire.
• Must be able to write, speak and read English proficiently.
• Ability to interact with patients and caregivers and healthcare team.
• Position requires long periods of standing and walking.

Responsibilities

• Identify the patient to be seen per hospital policy based on alerts and consults.
• Verify and document patient allergies and reported reactions.
• Complete medication history for each medication identified using a questionnaire to obtain a comprehensive list.
• Document each medication in the electronic medical record (prescription, inpatient/outpatient meds, OTC, supplements).
• Document dates and times of last doses per protocol.
• Screen for vaccination history.
• Confirm medication history using secondary sources (outpatient pharmacy, MARs, etc).
• Knowledge of hospital policies and compliance.
• Training and precepting other technicians, pharmacists and students.
