Job summary

Qualifications

• Registered with the Iowa Board of Pharmacy at time of hire.
• National certification through PTCB or NHA with ongoing CE requirements.
• High school diploma or equivalent is required.
• Four years of post-high school science experience or four years as a pharmacy technician.

Responsibilities

• Triage patients for medication history collection based on complexity.
• Use EMR tools to identify patients for history completion.
• Coordinate with pharmacist staff on patient histories.
• Collect data to complete admission medication histories for pharmacist reconciliation.
• Independently author medication history chart notes in the EMR.
• Communicate status of histories to pharmacists verbally or in writing.
