Qualifications

• Bachelor’s or Master’s Degree in Social Work.
• Licensure or license-eligible with Alabama State Board.

• Strong organizational and time management skills.

Responsibilities

• Provide direct social work services to a caseload of clients across levels of care.
• Complete biannual psychosocial assessments and case plans.
• Collaborate with medical team to address medical needs and access to care.
• Maintain HIPAA compliance and thorough documentation in EHR.
• Lead or participate in multidisciplinary health teams and trainings.
• Supervise staff and coordinate transportation and ARAP-related duties.
