Job summary

Viva Health in Montgomery, AL seeks a Social Worker, Care Manager, Behavioral Health. This full-time role supports members to achieve optimal wellness with a focus on psychosocial health and community resources.

You will collaborate with a multidisciplinary team, assist with utilization management, and conduct fieldwork outside the office. Travel within the service area is required; MSW with LMSW license preferred.

Qualifications

• MSW degree and 3 years of social work experience.
• Current LMSW license in good standing with Alabama.
• Valid driver's license in good standing.
• Willingness to submit to vaccine testing and screening.
• Background in behavioral health.
• Willingness to travel within Viva Health service area.

Responsibilities

• Evaluate member needs to achieve optimal wellness.
• Guide members and families to appropriate care resources.
• Collaborate with multidisciplinary team on psychosocial health.
• Support behavioral health panels for utilization management.
• Perform fieldwork outside the office as needed.
• Promote treatment plan adherence with providers.
• Travel to service area locations as required.
