Job summary

Signature Performance seeks a Medical Coding Specialist to assign ICD-10-CM/PCS, CPT and HCPCS codes from medical records, ensuring accurate documentation and coding compliance. You will review discharge summaries, progress notes, labs and operative records to assign diagnoses and procedures and determine appropriate reimbursement.

Experience with professional fee coding and EHR systems is required, with AHIMA or AAPC credentials strongly preferred.

Qualifications

• Minimum 2 years of Medical Coding experience.
• Experience with Professional Fee Coding.
• Experience with EHR systems.
• Certifications from AHIMA or AAPC are required or preferred: RHIT, RHIA, CCA, CCS, CCS-P, CPC.

Responsibilities

• Assign principal and secondary diagnoses and procedures by reviewing documentation.
• Review records to determine diagnoses and procedures and assign codes.
• Assigns and ensures correct code selection following Official Coding Guidelines.
• Ensure diagnoses/procedures are sequenced to determine DRG/APC for reimbursement.
• Review coding for accuracy and completeness prior to submission.
• Abstract data from medical records and correct errors in the system.
• Identify incomplete or conflicting documentation and formulate physician queries.
