Chase City Health & Rehab Center in Chase City, VA is seeking an experienced MDS Coordinator RN/LPN to lead the MDS/RAI process and coordinate care planning.

This full-time role requires VA RN license, MDS 3.0 expertise, RAC-CT within 1 year, strong computer skills, and the ability to work under pressure.

Qualifications

• Current RN license in VA.
• MDS 3.0 experience required.

Responsibilities

• Provide leadership to interdisciplinary team for MDS/RAI processes and reimbursement.
• Ensure MDS deadlines and coding accuracy for optimal reimbursement.
• Support Quality Improvement, staff education, and regulatory compliance related to RAI.
• Foster a positive, collaborative, and patient-centered team environment.
