Job summary

Ciena Healthcare is seeking an experienced MDS Coordinator (RN) to oversee the RAI process, conduct assessments, and coordinate care plans for residents. The role includes supervision of the Care Management Nurse and MDS Nurse.

The MDS Coordinator will work with the interdisciplinary team to ensure timely MDS and PPS compliance, staying current with AANAC requirements and Medicare schedules.

Qualifications

• Knowledge of the Resident Assessment Instrument (RAI) process and PPS.
• Experience as an MDS Nurse or MDS Coordinator.
• RN license and eligibility to practice nursing.

Responsibilities

• Completes the MDS, CAAs, and care plans within regulated time frames.
• Coordinates scheduling of the RAI process with the interdisciplinary team.
• Assesses residents through physical assessment, interviews, and chart reviews.
