Ciena Healthcare in Michigan is seeking an experienced MDS Coordinator to oversee the RAI process and coordinate care plans for residents. The role supervises the Care Management Nurse and MDS Nurse, ensuring timely MDS, care plans, and PPS activities.

The position emphasizes collaboration with the interdisciplinary team, adherence to PPS timelines, and ongoing knowledge of AANAC requirements. A meaningful sign-on bonus and comprehensive benefits are offered.

Formación

• RN license required with knowledge of RAI process; PPS knowledge strongly preferred.
• Experience as an MDS Nurse is required or preferred.

Responsabilidades

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Prepares PPS meetings and maintains PPS board for monitoring Medicare days and RUGs utilization when needed
• Stays current with AANAC requirements.
