Job summary

Ciena Healthcare is seeking an experienced MDS Coordinator in Michigan to oversee the RAI process and coordinate comprehensive care plans for residents. You will supervise the Care Management Nurse and the MDS Nurse while collaborating with an interdisciplinary team to ensure high-quality, compliant care.

The role requires in-depth knowledge of PPS and AANAC standards, timely assessments, and meticulous care planning.

Qualifications

• RN licensure in good standing.
• Strong knowledge of the Resident Assessment Instrument (RAI) and PPS concepts.
• Experience as an MDS Nurse or similar RN leadership in long-term care.

Responsibilities

• Completes MDS, CAA’s and care plans within required regulatory timelines.
• Coordinates scheduling of the RAI process with the interdisciplinary team.
• Assesses residents through physical exams, interviews and chart reviews.
• Discusses resident care needs with physicians, nursing, social services, therapy, dietary and activities staff.
• Reviews hospital data, consults and external information to inform assessments and care plans.
• Initiates significant changes to MDS as needed.
• Maintains PPS board and conducts PPS meetings in coordination with the Care Management Coordinator; adheres to Medicare schedules.
• Stays current with AANAC requirements.
