Job summary

Ciena Healthcare is seeking an experienced MDS Coordinator to oversee the RAI process and care plan coordination for residents. The role supervises the Care Management Nurse and MDS Nurse, working within a team that values communication and teamwork.

Responsibilities include completing MDS/CAA and care plans, coordinating PPS meetings, and ensuring compliance with Medicare schedules and AANAC requirements. RN licensure and MDS experience are required.

Qualifications

• Registered Nurse with current license
• Knowledge of the RAI process and PPS principles preferred
• Experience as an MDS Nurse is required

Responsibilities

• Completes the MDS, CAAs and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team
• Assesses residents through physical exams, interviews and chart review
• Discusses resident care needs with care givers including physicians, nursing, social services, therapy, dietary and activities staff
• Reviews information from hospitals, consults and outside agencies to complete assessments and care plans
• Identifies and initiates significant changes to MDS records
• Prepares PPS meetings and maintains the PPS board for Medicare days and RUGs

Completes the MDS, CAA’s and care plans within regulated time frames.

Coordinates scheduling the RAI process with the interdisciplinary team

Assesses resident through physical assessment, interview and chart review.

Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.

Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.

Coordinates, identifies, and/or initiates significant change MDS’

Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator

Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

Registered Nurse (RN)

Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
