Centene is seeking a Senior LTSS Service Care Manager RN - Pediatrics in San Antonio, TX to assess, plan and coordinate care for high-acuity members. The role emphasizes evidence-based care management and collaboration with families and providers.

The position requires an RN license (or NP), 4–6 years of related experience, and RUG certification within 90 days of hire. Flexible schedules include remote or hybrid arrangements with competitive pay.

Formación

• Graduate from an Accredited School of Nursing or Bachelor's degree required.
• Bachelor's degree in Nursing preferred.
• 4-6 years of related experience.
• RN licensure and/or NP licensure required, or
• RUG certification must be obtained within 90 days of hire.

Responsabilidades

• Evaluate the service needs of complex or high-risk members and recommend a plan for the best outcome.
• Develop and continuously assess ongoing long-term care plans/service plans.
• Coordinate and manage care between the member, family/caregivers, and care provider team.
• Monitor care plans/service plans and member status, revising as necessary.
• Monitor member status for complications and clinical symptoms.
• Review member data to identify trends and improve quality care.
• Review referrals and intake assessments to develop appropriate care plans.
• Collaborate with healthcare providers to facilitate member services and treatments.
• Collect, document, and maintain member information and care management activities.
• Provide education to long-term care members and their families on healthcare options.

About This Role

The Senior LTSS Service Care Manager RN - Pediatrics is responsible for performing care management duties to assess and coordinate medical and supporting services for complex/high acuity populations. This role is ideal for experienced RNs who are passionate about promoting quality, cost-effective care.

• Pay: $36 - $65/hour
• Location: San, US
• Schedule: Full Time, Day shift
