Job summary

DRISCOLL HEALTH PLAN seeks a Service Coordinator III to lead person-centered care coordination for LTSS and STAR Kids members. You will review services, coordinate with physicians and family, and support the care team to assure timely, community-based care.

Ideal candidates will have background in case management, managed care, and Texas Medicaid guidelines, with a focus on confidentiality and regulatory compliance within a health system.

Qualifications

• Experience as a case management support technician or LTSS coordinator preferred.
• High school diploma or equivalent required.
• At least a 2-year college Associate’s Degree preferred.
• Previous experience in a managed care environment preferred.
• Direct experience with STAR Kids or similar programs preferred.

Responsibilities

• Review member’s services and supports needs in collaboration with the assigned service coordinator.
• Coordinate communications with physicians, families, caregivers and the health care team.
• Staff phones for the Service Coordination Pod as assigned.
• Support care coordination administratively for RN and SW teams.
