Job summary

Driscoll Health System in South Texas is seeking a Service Coordinator III to advance person-centered care coordination for pediatric members. You will review services, collaborate with assigned coordinators, and support LTSS access to prevent institutionalization while keeping members safe at home.

Responsibilities include staff phone coverage for the Service Coordination Pod, coordination with physicians, families, and care teams, and ensuring timely, compliant care plans within STAR Kids

Qualifications

• Experience as a case management support technician or LTSS coordinator preferred.
• 2-year college Associate’s Degree preferred.
• Three of the last five years working with similar conditions if contractual Level 3.
• Minimum 3 years of clinical experience (pediatric preferred) or 1 year in managed care.

Responsibilities

• Care coordination and administrative support for the assigned Service Coordination Pod.
• Review member services and supports needs with the assigned coordinator for LTSS access.
• Interact with physician offices, families, and care teams to coordinate care.
• Staff the phones for the Service Coordination Pod and monitor communication.
