Job summary

Signature HealthCARE of McCreary County Rehab & Wellness Center is seeking a compassionate LPN/LVN to join our nursing team in Kentucky. You will provide direct nursing care, administer medications, perform wound care, and assist residents with daily activities to maintain comfort and safety.

You will supervise nursing assistants, document care, coordinate admissions and discharges, and participate in quality assurance activities with a collaborative interdisciplinary team.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role is a plus.
• Strong observation, medication administration, and documentation skills.
• Familiarity with federal and state long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity and safety.
• Supervise and direct nursing assistants and medication aides during shifts per policies.
• Monitor residents' status and report changes to physicians or care team.
• Develop, implement, and review individualized resident care plans.
• Complete accurate clinical documentation and coordinate admissions, transfers and discharges.
• Participate in quality assurance activities and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing.
