Signature HealthCARE in Liberty, KY is seeking an experienced LPN/LVN to deliver direct nursing care, manage medications, and supervise nursing assistants on the shift. You will collaborate with the interdisciplinary team to develop care plans, ensure patient comfort and safety, and maintain accurate documentation in the EHR.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and Microsoft Office.

Responsabilités

• Provide direct nursing care to residents, including medication administration and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring dignity and safety.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor residents’ condition and notify physicians of changes.
• Develop and update individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
