Job summary

Signature HealthCARE is seeking an experienced RN for McCreary County Rehab & Wellness Center in McCreary County, KY. you will provide direct nursing care, oversee nursing staff, and ensure high-quality resident outcomes in a 60-bed facility.

Qualifications

Current, active RN license in the state of practice.
CPR/BLS certification preferred.
Minimum one year of clinical nursing experience; long-term care preferred.
Experience in a supervisory or charge nurse role preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Effective verbal and written communication skills.
Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
Familiarity with electronic health records (EHR) systems.

Responsibilities

Provide direct nursing care to residents, including medication administration and wound care.
Assist residents with activities of daily living (ADLs) with dignity and safety.
Supervise and direct nursing assistants and medication aides during shifts.
Monitor residents’ status and notify physician/responsible party of changes.
Develop and review individualized resident care plans with the team.
Complete accurate clinical documentation per policy.
Coordinate admissions, transfers, and discharges per procedures.
Participate in quality assurance activities and staff education.
Perform other duties as assigned by the Director of Nursing.
