Job summary

Livingston Place Assisted Living is seeking a dedicated Social Worker to oversee social services for residents in the DC area. The role emphasizes assessment-driven planning, emotional support, and coordination with an interdisciplinary team to enhance resident well-being.

Ideal candidates will have an Associate or Bachelor’s in Social Work, with at least two years in senior living and knowledge of HCBS Medicaid programs. LSW preferred, on-site position in Washington, DC.

Qualifications

• Associate or Bachelor’s degree in Social Work required; LSW preferred.
• Minimum two years of experience in senior living.
• Knowledge of HCBS Medicaid program.
• Experience in a social service role within long-term care or healthcare setting.

Responsibilities

• Provide leadership and direction for the social services department to ensure high-quality resident services.
• Conduct comprehensive assessments and develop individualized care plans addressing social, emotional, and cultural needs.
• Offer emotional support, assist with government program qualification, and provide counseling as needed.
• Collaborate with nurses, therapists, physicians, case workers, and other staff to coordinate resident care.
• Maintain documentation, stay informed on regulations, and participate in interdisciplinary team meetings.
