Qualifications

• Knowledge and experience with HCBS Medicaid Program.
• Proven experience in a social service role within a long-term care or healthcare setting.

Responsibilities

• Provide leadership and direction for the social services department and needs of the community.
• Conduct comprehensive assessments of residents' psychosocial needs and develop individualized care plans.
• Offer counseling and guidance to residents and families, advocating for HCBS and government programs.
• Collaborate with nurses, therapists, physicians, and case workers to ensure holistic resident care.
• Take meeting minutes and maintain documentation in PCC; monitor waiver renewal dates.
