Job summary

Gilbane Development Company is seeking an Assisted Living Social Worker to oversee social services for residents at Livingston Place Assisted Living in DC. The role emphasizes leadership, coordination with healthcare professionals, and ensuring high-quality, person-centered care.

The ideal candidate has a social work degree, a DC license, 5+ years in long-term care or healthcare settings, and experience with HCBS Medicaid. Organization, empathy, and teamwork are essential in this role.

Qualifications

• Minimum of Associate or Bachelor's in Social Work required.
• DC Social Work license required; LGSW or LICSW preferred.
• Minimum of 5+ years in a social service role within long-term care or healthcare.
• Experience with HCBS Medicaid Program.
• Experience fostering partnerships with Assisted Living resident agencies.

Responsibilities

• Provide leadership and direction for the social services department and needs of the community.
• Conduct comprehensive assessments of residents' psychosocial needs and develop care plans.
• Advocate for residents and assist with government program qualifications (HCBS, Food Stamps).
• Collaborate with the interdisciplinary team to address complex social issues affecting residents.
• Maintain detailed records and ensure regulatory compliance within PCC and DC Care Connect.
