Job summary

The Suites at Someren Glen in Centennial, CO is seeking an RN Care Transitions Manager to oversee transitions of residents from skilled nursing to home, coordinating education, care planning, and interdepartmental communication. You will guide residents and families, ensure safety, and promote successful aging practices.

We value strong nursing leadership, regulatory expertise (PPS, MDS, OBRA, OSHA), and collaborative, team-oriented care.

Qualifications

• Active RN license in good standing.
• BSN preferred.
• 2+ years of related nursing experience as RN or LPN.
• 1-3 years in a supervisory capacity.
• Colorado regulatory knowledge for skilled nursing facility operation.

Responsibilities

• Coordinate work to meet resident goals and expectations.
• Perform post-admission screening to gauge knowledge of diseases.
• Coach residents to improve activation levels and care plans.
• Maintain care conferences, updates, and discharge planning.
• Assist with quality improvement and QAPI initiatives.
• Support training and supervision of team members.
